Коленный сустав является самым крупным и сложным по строению и биомеханике суставом, в связи с этим наиболее уязвимым звеном опорно-двигательной системы. На долю травм коленного сустава приходится около 50 % всех травм суставов в теле человека. Наиболее часто встречаются повреждения капсульно-связочного аппарата коленного сустава, что определяет актуальность данной проблемы. Своевременное распознавание наличия и степени тяжести патологии крестообразных связок важно в определении выбора лечебной тактики, включая их хирургическую реконструкцию.</p></sec><sec><title>   Цель исследования</title><p>   Цель исследования. Определение возможности диагностического применения метода спектроскопии диффузного отражения для выявления несостоятельности крестообразных связок коленного сустава.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Объектом исследования являлись экспланты крестообразных связок коленного сустава, полученные у пациентов, которым выполнялось плановое первичное тотальное эндопротезирование коленного сустава. Забор осуществляли вместе с костными блоками в местах прикреплений крестообразных связок. В предоперационном периоде всем пациентам выполняли магнитно-резонансную томографию с оценкой состояния крестообразных связок по шкале ACLOAS. Состоятельность связок оценивали до забора, используя классификацию AAOS. Впоследствии экспланты подвергали исследованию с помощью метода спектроскопии диффузного отражения в ближнем (800–1050 нм) и среднем (1050–1800 нм) инфракрасных диапазонах.</p></sec><sec><title>   Результаты</title><p>   Результаты. В ближнем инфракрасном диапазоне при длине волны 910 нм обнаружены статистически значимые различия (p &lt; 0,05) между полученными показателями по всем трем степеням тяжести поражений связок в соответствии с классификацией AAOS. В среднем инфракрасном диапазоне в диапазоне 1200–1050 нм зафиксирована статистически высокозначимая разница между средними показателями I и II степени поражений связки по AAOS (p &lt; 0,0001).</p></sec><sec><title>   Заключение</title><p>   Заключение. Анализ данных показал, что спектроскопия диффузного отражения света является перспективным диагностическим инструментом для дифференцировки стадий патологии крестообразных связок коленного сустава.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. The knee joint is the largest and most structurally and biomechanically complex joint, which makes it the most vulnerable component of the musculoskeletal system. Injuries to the knee joint account for approximately 50% of all joint injuries in the human body. Damage to the capsuloligamentous apparatus of the knee joint is the most common, which determines the relevance of this problem. Timely recognition of the presence and severity of cruciate ligament pathology is essential for determining the optimal treatment strategy, including surgical reconstruction.</p></sec><sec><title>   Aim of the study</title><p>   Aim of the study. To assess the feasibility of using diffuse reflectance spectroscopy as a diagnostic method for detecting insufficiency of the knee joint cruciate ligaments.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The study objects were explanted cruciate ligaments of the knee joint obtained from patients undergoing elective primary total knee arthroplasty. Tissue harvesting was performed together with bone blocks at the sites of cruciate ligament attachment. In the preoperative period, all patients underwent magnetic resonance imaging with assessment of cruciate ligament status using the ACLOAS scale. Ligament integrity was evaluated prior to harvesting using the AAOS classification. Subsequently, the explants were examined using diffuse reflectance spectroscopy in the near-infrared (800–1050 nm) and mid-infrared (1050–1800 nm) spectral ranges.</p></sec><sec><title>   Results</title><p>   Results. In the near-infrared range at a wavelength of 910 nm, statistically significant differences (p &lt; 0.05) were identified among the measured parameters for all three grades of ligament injury severity according to the AAOS classification.
