На сегодняшний день необходимость замещения дефектов костной массы проксимального отдела плечевой кости (ПОПК) при выполнении тотального реверсивного эндопротезирования плечевого сустава является обсуждаемым вопросом. В связи с чем возникла необходимость изучить возможности использования индивидуально изготовленных 3D аугментов.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: анализ результатов оперативного лечения с применением индивидуально изготовленных 3D аугментов ПОПК и сравнение их с результатами традиционных методов оперативного лечения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: В исследование были включены 90 пациентов с костными дефектами ПОПК, которым было выполнено тотальное реверсивное эндопротезирование плечевого сустава. В исследовании выделялось 2 группы пациентов: 1 группа - 46 (51,1%) пациентов, которым выполнено оперативное вмешательство с применением индивидуально изготовленных 3D аугментов проксимального отдела плечевой кости, 2-ая группа - 44 (48,9%) пациента, которым выполнено оперативное вмешательство по традиционной методике. Анализу подвергались такие данные как общее время операции, инраоперационная кровопотеря, количество послеоперационных осложнений, а также функциональные результаты лечения, основанные на данных шкал-опросников СКИ, DASH и ВАШ. Средний срок наблюдения составил 14,8 месяцев.</p></sec><sec><title>Результаты</title><p>Результаты: отмечено статистически значимое (р&lt;0,05) уменьшение среднего времени операции, инраоперационная кровопотеря и количества послеоперационных осложнений в 1-й группе пациентов. Анализ данных шкал-опросников также показал статистически значимые (p&lt;0,05) лучшие функциональные результаты лечения в 1-й группе пациентов.</p></sec><sec><title>Заключение</title><p>Заключение: По итогам исследования можно заключить, что при наличии выраженного дефекта ПОПК использование 3D индивидуальных аугментов имеет множество преимуществ, таких как: меньшее среднее время операции и интраоперационная кровопотеря, а также меньшее количество осложнений. Также следует отметить, что аугментация ПОПК обеспечивает лучшие функциональные результаты лечения, стабильность в суставе и является профилактикой вывихов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction.</p><p>To date, the need to replace bone mass defects of the proximal humerus when performing total reverse shoulder arthroplasty is a debated issue. Therefore, it became necessary to study the possibilities of using personalized 3D augments of the proximal humerus. Purpose of the study: to analyze the results of surgical treatment with the use of personalized 3D augments of the proximal humerus and to compare them with the results of traditional methods of surgical treatment.</p></sec><sec><title>Materials and methods</title><p>Materials and methods:</p><p>There were 90 patients with bone defects of the proximal humerus who underwent total reverse shoulder joint arthroplasty were included in the study. The study distinguished 2 groups of patients: Group 1 - 46 (51.1%) patients who underwent surgical treatment using custom-made 3D augments of the proximal humerus, Group 2 - 44 (48.9%) patients who underwent surgical treatment using the traditional technique. Such data as total operation time, intraoperative blood loss, number of postoperative complications, as well as functional results of treatment based on the data of SCI, DASH and VAS scales were analyzed. The average follow-up period was 14.8 months.</p></sec><sec><title>Results</title><p>Results:</p><p>statistically significant (p&lt;0.05) reduction of the average operation time, intraoperative blood loss and the number of postoperative complications in the 1st group of patients was observed. Analysis of the scale-questionnaire data also showed statistically significant (p&lt;0,05) better functional results of treatment in the 1st group of patients.</p></sec><sec><title>Conclusion</title><p>Conclusion: </p><p>According to the results of the study we can conclude that in case of the proximal humerus bone loss the use of 3D customized augments has many advantages, such as: shorter average operative time and intraoperative blood loss, as well as fewer complications.
