Патогенетические и технические аспекты радикальных вмешательств на суставах при анкилозирующем спондилоартрите

Аннотация

Последние десятилетия ортопедическая наука выделила ряд фундаментальных направлений в восстановлении функциональных возможностей пациентов, основанных на применении высокотехнологичных методов лечения патологии опорно-двигательной системы. Артропластика стала ключевым элементом в решении проблем поздних стадий заболеваний суставов, особенно нижних конечностей. Однако, системная патология, в том числе аутоиммунные заболевания скелета, особенно ревматоидный артрит (РА) и анкилозирующий спондилоартрит (АС), делают уникальные вызовы, требующие специализированного подхода. Значение эндопротезирования по замене тазобедренного и коленного сустава в комплексном лечении АС невозможно переоценить. Радикальные вмешательства позволяют облегчить боль, восстановить функцию суставов и улучшить качество жизни столь сложной группы пациентов. Они играют важную роль в восстановлении физической активности пациента и смягчении последствий тяжелой патологии скелета. Не решая коренной задачи исцеления, метод способствует повышению качества жизни пациента, что делает его на сегодня незаменимым. Цель работы - рассмотреть варианты лечения и провести анализ публикаций отечественных и зарубежных авторов, посвященные вопросам анкилозирующего спондилоартрита с акцентом на хирургическую коррекцию поздних стадий суставных проявлений. Методология поиска источников. При подготовке обзора литературы использованы электронные базы данных научной литературы PubMed (MEDLINE) и eLibrary.Ru Поиск данных литературы произведён по следующим ключевым словам: анкилозирующий спондилоартрит, реактивный артрит, эндопротезирование, ankylosing spondylitis, rheumatoid arthritis, endoprosthetics использовали следующие КРИТЕРИИ ВКЛЮЧЕНИЯ: рандомизированные контролируемые клинические исследования, клинические случаи, систематические обзоры, метаанализы. КРИТЕРИЯМИ ИСКЛЮЧЕНИЯ являлись статьи без полнотекстовой версии, дублирующие публикации. Предпочтение отдавали работам за последние 5-10 лет.
