Рефиксация корня медиального мениска. Оценка послеоперационных результатов и влияние экструзии на клинический результат

Аннотация

Введение. Повреждение корня медиального мениска частая патология коленного сустава, которая может привести к полной потери функции медиального мениска (ММ) и значительным дегенеративным изменениям в коленном суставе. Примечательным является остаточная экструзия ММ на послеоперационной МРТ. Ряд авторов не нашли корреляции между исходом операции и функциональным результатом.

Цель работы оценка субъективных и объективных результатов лечения пациентов с разрывом корня медиального мениска коленного сустава после рефиксации заднего корня медиального мениска транстибиальной модифицированной техникой.

Материалы и методы. Проспективное нерандомизированное исследование включает 32 пациента с разрывом корня ММ, которым выполнена рефиксация корня с применением модифицированной транстибиальной техники фиксации. Все пациенты женщины 43–72 лет, ни у одной пациентки хондромаляция не превышала 2 ст. по классификации ICRS. Функциональное состояние оценивали по KOOS и ВАШ, а также выполняли МРТ с оценкой эксрузии мениска и степени сращения корня ММ до и после операции. Срок наблюдения 12 мес.

Результаты. У 28 пациенток (87,5 %) отмечено полное заживление, а в 4 (12,5%) – частичное заживление. Показатель болевого синдрома снизился с (7,7±0,7) балла по ВАШ до (1,8±1,2) балла. Средний показатель по шкале KOOS улучшился с (38,6±12,5) баллов до (75,6±14,4) баллов. Средняя величина экструзии мениска до операции составила (4,06±0,82) мм, после операции – (2,67±0,71) мм, что явилось статистически значимым.

Заключение. Рефиксация корня медиального мениска, выполненная с применением модифицированной техники, позволила добиться хороших результатов в 75,0 % наблюдений у пациентов c разрывом корня медиального мениска коленного сустава. Степень экструзии ММ после артроскопической фиксации его корня показала умеренную тесноту связи с функцией коленного сустава. При варусной деформации нижней конечности со значением угла НКА <175° функциональные результаты статистически значимо ухудшаются, и оцениваются как неудовлетворительные.
