Результаты лечения переломо-вывихов локтевого сустава с применением авторского фиксатора: ретроспективное когортное исследование

   Лечение переломовывихов предплечья и наружной латерально-ротационной нестабильности локтевого сустава является сложной задачей. Авторами предложено динамическое фиксирующее устройство, изготовляемое во время операции, которое позволяет улучшить результаты лечения данной патологии.

   Цель исследования: изучить эффективность применения авторского способа оперативного лечения и специального имплантата при лечении переломовывихов костей предплечья.

   Материалы и методы: Проведен ретроспективный анализ результатов лечения пациентов с закрытыми изолированными переломовывихами локтевого сустава в период с 2009 по 2019 гг., в исследование включено 23 пациента (14 женщин и 9 мужчин), среднего возраста 47,8 ± 12,1 лет. Пациенты были оперированы с применением авторского динамического стабилизатора средний предоперационный период составил 7,6 ± 3,9 суток.

   Результаты. Болевой синдром по шкале ВАШ до операции составлял 5,9 ± 1,0 балла, в первые сутки после операции увеличивался до 7,8 ± 0,9 балла и затем постепенно медленно снижался к 14 суткам после операции до 2,0 ± 0,7 балла. Динамометрия силы кисти в 3 месяца (35,8 ± 5,0 кг) к 12 месяцам (55,8 ± 7,1 кг) увеличилась на 35,8 %. Результаты по шкале MEPS в сроки с 1,5 и до 12 месяцев, вырос с 49,7 ± 4,4 до 87,7 ± 8,9 баллов на 43,3 %, шкале DASH в 1,5 месяца составил 31,8 ± 10,3 балла и в 12 месяцев снизился до 18,5 ± 10,2 баллов (на 41,8 %).

   Вывод: при лечении переломовывихов предплечья эффективно раннее хирургическое вмешательство с применением оригинальной металлоконструкции, которые позволяют начать раннюю реабилитацию, не применяя иммобилизации, получать хорошие функциональные результаты, избегая формирования посттравматических нестабильности и контрактур локтевого сустава.
